Nota del Dr. F. Hernández Altemir: Estando saliendo de una lesión en la Cabeza  humeral derecha, que duró unos dos años,  cuando pertenecía al Club del Estudiantes, en el Ramiro de Maeztu, acudi a un Cursillo  que había organizado  si recuerdo bien,  la Federación Española de Baloncesto, en Madrid y fué allí, cuando, me vió entrenar Pedro Ferrandiz y sin más, al poco tiempo, recibí una llamada, para preguntarme, si me gustaría fichar por el HESPERIA, filial del Real Madrid de Baloncesto y entrenar por las noches, alrededor de las 22 horas. en el Frontón Fiestalegre, detrás, del Edificio de Correos, de la Plaza de Civeles y  actual Ayuntamiento de Madrid, porque  se estaba formando, en aquel momento. 
Un servidor, no tenía equipo y le dije, al mencionado, que si y me preguntó, cuanto quería cobrar, de fichaje, yo le dije, mira, necesito unas gafas, sobre todo, para cuando jugamos por la noche y me las pagaron….., también me dijo, que no iba a jugar mucho, que solo me fichaba, para que le espabilara a un Pivot, de más de dos metros……. que venía de la Armada de Ciudad Lineal (mis fintas, incluso, no pocas veces hacían saltar a los que nos arbitraban  e incluso a  los Espectadores…..) fuí compañero de Pablo Laso, de Sevillano, Crespo ……, a veces F. (disculpa Pedro) se enfadaba conmigo, porque en la repetición de jugadas de los entrenamientos, no pocas veces, cortaba la trayectoria del balón…..también lo hacia, por mis  «contraataques», que me indicaba los hiciera mas rápidos y contundentes, le contestaba, que mi madre, me habia dicho, que tuviera cuidado, no me diera un golpe fuerte…

Abstract: The aim of this study was to analyse and compare the position of single‐rooted autotransplanted teeth using computer‐aided SNT drilling and conventional freehand (FT) drilling, by comparing the planned and performed position at the coronal, apical and angularlevel. Materials and methods: Forty single‐root upper teeth were selected and distributed into the following study groups: A. Autotransplanted tooth using the computer‐aided static navigation technique (SNT) (n = 20) and B. Autotransplanted tooth using the conventional free‐hand technique (FT) (n = 20). Afterwards, the teeth were embedded into two experimental models and 10 single‐root upper teeth were randomly autotransplanted in each experimental model. The experimental models were submitted to a preoperative cone‐beam computed tomography (CBCT) scan and a digital impression by a 3D intraoral scan, in addition to a postoperative CBCT scan, after the autotransplantation. Datasets from postoperative CBCT scans of the two study groups were uploaded to the 3D implant planning software, aligned with the autotransplantation planning, and the coronal, apical and angular deviations were measured. The results were analysed using Student’s t‐test and Mann–Whitney non‐parametric statistical analysis. Results: Coronal (p = 0.079) and angular (p = 0.208) statistical comparisons did not present statistically significant differences; however, statistically significant differences between the apical deviation of the SNT and FT study groups (p = 0.038) were also observed.
Conclusions: The computer‐aided static navigation technique does not provide higher accuracy in the positioning of single‐root autotransplanted teeth compared to the conventional free‐hand technique.

Implant-supported Occlusal Platform

Abstract:

Purpose: The purpose of this work is to design Oclusal Platforms with the aim of avoiding complications derived from over-implanted prostheses. They are prone to create certain issues in nearby areas, gingival stomatitis, mucositis, etc. To prevent those, some complex hygienic measures are required. This results in complex practices for the patiente, often of advanced age. Therefore, it finally results in frequent visits to the specialists (from Dentists to Maxillofacial surgeons, etc.). Methods Treatments with implanted and mixed prostheses are common and bring enormous benefits. However, they also require special hygienic measures (frequently complex for advanced age patientes). To avoid previously described complications, we propose our Oclusal Platforms implant-supported or even mixed-dental-supported. One Platform could depend from the superior jaw and other from the mandibular that would surely occlude perfectly amongst each other. Results Making of implant-supported prostheses is a conventional process simply adapting the platforms to the specific characteristics of each patient. Generally speaking, this could be from a toothless patient to a partially toothless patient in one of both jaws. The platform is then placed so that the chewing capacity is recovered in first place. Secondly, it would be added together with the platform a sketch of incisors and canines or the pieces deemed necessary without reaching the impanted gingival area. Conclusions The occlusal deviced proposed is simple and applicable in a way that the patiente recovers the funcional capacity for chewing, challowing and speaking, without forgetting the aesthetic aspects.

Esperando se encuentre todos bien, le remito para Referenciarlo para la Propiedad Intelectual el Titulado:
 
TRAQUEOSTOMIA PREFORMADA  LOS TIEMPOS DE COVID-19 y OTROS
La incidencia del Covid-19, esta llevando a que muchos pacientes, tengan que ingresar en Unidades  de Emergencia. incluso improvisadas y que son atendidas, no pocas veces por personal sanitario, que se aparta, de los comunes de las UVIS TRADICIONALES o de las SALAS DE REANIMACION, lo que nos ha hecho pensar, que para determinados casos, de pacientes con posibilidad, de tener que realizarles Traqueostomías, se podria tener en consideración, la  TRAQUEOSTOMIA PREFORMADA, para así ayudar al Personal Sanitario,  cuando se diera el caso, de tener que llevar a efecto una Traqueostomía convencional o más o menos, urgente.
 
Lo habitual, es que en este tipo de tratamiento de permiabilización de la vias aereas, la Traqueostomía, ocupe un lugar preferente y suelen llevarla a efecto en condiciones de normalidad asistencial, Intensivistas y/o incluso, Anestesiólogos, en momentos, de más o menos inmediatez.
 
Los Especialistas de Patologías Quirúrgicas, de Cabeza y Cuello, esto es,   Otorrinolaringólogos, Cirujanos  Orales y Maxilofaciales y Cirujanos Plásticos, etc., no pocas veces, son requeridos para traqueotomizar, en más o menos programadas, por las Unidades, antes referidas o  ahora en estos tiempos, ya  por Internistas y Generalistas,  Médicos de Urgencias y un gran etc.
 
En la situación de Pandemia que vivimos,  el personal saniario, de Especilidades, no pocas veces alejadas de las que las que habitualmente las componen, deben que tener que llevar a efecto actuaciones, para los que sus conocimientos y habilidades, no son todo lo precisas o que incluso siéndolas, son excesivamente numerosas o mas o menos dudosas, para tener, que llevarlas a efecto.
 
Nuestra propuesta, de TRAQUEOSTOMIA PREFORMADA,  deja preparada, la Región Traqueal con antelación, para que si por fin, hay que transformarla en una Traqueostomia convencional, el actor, vea facilitado su trabajo quirurgico y no sólo, porque  sea una técnica demasiado especializada, sino porque no pocas veces, su conocimiento y difusión,  podrían aliviar al personal de Uvis y/o Reanimaciones, para estos efectos y puedan actuar con mas celeridad e incluso, en no pocas ocasiones, evitar traqueostomias, con lo que ello representa,  tambien para los pacientes.

Cirugía oral y maxilofacial cubre todas sus plazas en la cuarta jornada del MIR 2020

..Redacción.
Cirugía oral y maxilogfacial ha agotado todas sus plazas en la cuarta jornada de elección MIR 2020. Se suma así a Cirugía plástica y a Dermatología que habían cubierto su oferta el segundo día del proceso. Cinco candidatos ocuparon las vacantes que restaban para alcanzar las 32 previstas. El candidato 2.100 ha escogido la última disponible.

Cardiología se ha quedado a las puertas de acabar sus existencias. Aún falta por cubrir dos de las 180 plazas disponibles. A día de hoy el 98,9% de las plazas disponibles ya están asignadas. Se queda a las puertas de completar su oferta Neurocirugía. Siguen disponibles seis de las 45 vacantes previstas.

Cinco candidatos ocuparon las plazas que restaban para alcanzar las 32 previstas de Cirugía oral en el MIR 2020

Otras nueve especialidades han cubierto ya al menos la mitad de su oferta. Aparato digestivo ya asignado el 77,9% de su oferta. Le siguen Anestesiología (71%), Urología (66,6), Oftalmología (65,6), Cirugía ortopédica y traumatología (56,2%), Neurología (55,8), Otorrinolaringología (54,5%), Oncioología médica (54,3%) y Cirugía pediátrica (50%).

Un total de 27 candidatos eligieron este jueves la especialidad de Medicina familiar y comunitaria. Durante los cuatro primeros son ya 70 residentes los que han escogido esta especialidad. De momento se ha cubierto el 3,15% de las 2.224 de las disponibles. Pediatría ha agotado el 49,7% de las plazas disponibles. Hasta la  fecha se han ocupado 239 vacantes de las 481 disponibles. Es decir se ha cubierto un 49,7% del total de la oferta.

Hasta la fecha, seis especialidades no han asignado ni una sola plaza. Se trata de Alergología, Análisis clínicos, Bioquímica clínica, Cirugía torácica, Farmacología clínica y Oncología radioterápica.

El dispositivo, que proponemos, permite entre otros, si es abriendo la subnasal, realizar las funciones habituales del mencionado apéndice, como sonarse y/o aplicar medicaciones y claro pruebas PCR, exploraciones ORL, etc.

La comisural bucal, abriendo de lado a lado, permite la alimentación oral y de la misma manera, la higiene bucal y de cuantas funciones es capaz la boca y todo, sin quitarse la mascarilla.

Para determinado tipo de Individuos  y/o Pacientes, el Sistema  de cremalleras o similares al efecto,  para las funciones antes someramente señaladas, también pueden ser verticales y únicas mediofaciales y desde la Región glabelar, hasta la Submental y para de parecida manera, evitar el quitarse la mascarilla y el encordio, que ello significa, en no pocas ocasiones.

Incluso Técnicas diversas de Intubaciones endotraqueales, pueden llevarse a efecto, tanto por vía oral, como nasal y Submental, sin retirar las mencionadas mascarillas, siendo válido el procedimiento, para tanto el  intra, como el extraoperatorio, en Quirófanos y en las Salas de Reanimación y/o de Intensivos.

Abstract
Submental intubation is used in patients with maxillofacial trauma to enable free intraoperative access to the dental occlusion and nasolabial complex. Orthognathic surgery is usually performed with nasotracheal intubation to prevent the tube from interfering with occlusion. We used submental intubation in 2 cases of orthognathic surgery in which nasotracheal intubation was impossible. One patient was a 17-year-old male with maxillary hypoplasia associated with left cleft lip and palate. He had a pharyngeal flap precluding nasal tube placement. The other patient was 18-year-old female with facial hamartoma and jaw deformity. The hamartoma obstructed her nasal cavity. The surgery proceeded uneventfully and there were no complications. Submental intubation is a useful method not only for patients with maxillofacial trauma, but also in selected cases of orthognathic surgery. Hospitalarios…..(para mantener el cuerpo a cuerpo…. tradicional: médico-enfermo.-.).

Si  Don Gregorio Marañón,  Don Carlos Jíménez Díaz, Don Vicente  Gilsanz y otros…., de mis Profesores, levantaran la cabeza, no me imagino, el susto, que tendrían…

¿Que pasaría, con la Silla, que  Don Gregorio, tenía para sus pacientes, cuando les hacia su Historia y Exploración Clínica (ANAMNESIS)—?.

¡Por favor, sí, para casos seleccionados de Videoconsultas y similares, pero no nos quiten, la SILLA Y LAS CAMILLAS, de exploración de TODA LA VIDA….

¿Que va a pasar, con el Fonendoscopio y el Martillo de reflejos (seguro, que más de un´Neumólogo y  Neurólogo, llorarán…..).

Hola de nuevo, Rosario
Ahora, de nuevo un pensamiento, después de enterarme, hace unos minutos, que un paciente de 88 años, se ha recuperado, después de haber estado intubado  y al final,  traqueotomizado.
Y por qué pienso esto?
Porque, quizás la zona traqueal,   es más afuncional e inerte, que la zonas aerodigestivas superiores y por lo tanto, más capaces de superar, las complicaciones que las mismas representan, al ser más dinamicas y sensibles, a la presencia de elementos anómalos en y extraordinarios por extraños y más adjetivos…, por tanto, tratan de rechazarlos, también por superorganicos y teflexogenos,  con todo lo que lleva, en cuanto a la facilidad, para su control, tanto  material, como incluso farmacológico (necesidad de medicaciones muy precisas y de difícil control,  con incluso influencias globales, no siempre bien definidas, para cada caso y situación) y funcionalmente, por que su irrupción y perseverancia  actúan claramente, como un cuerpo extraño, casi intolerable… y que requiere, además grandes recursos asistenciales, de todo tipo…
La traquea, pasa más de estos aspectos, por ser más inerte e insensible fisiológica y materialmente a los mencionados tubos y similares,  menos reflexogena, menos productiva de secreciones y de más fáciles cuidados y atenciones, incluso presenciales y un gran, etc.
Para el Covid-19, lo mejor, parece la fisioterapia y el prono y si se Requieren intubaciones largas, la traqueostomia…..(aunque,  hay que seguir estudiando, con meticulosidad, ósea con la evidencia, sobre lo señalado….)

Asunto: Transplante de tejidos y/o folículos pilosos de pacientes, que han superado el Covid-19, quizás principalmente jóvenes (para Recepcionarlo)

Ya hace unos meses, le remiti, sobre el empleo principalmente de fluidos diversos de pacientes que hubieran superado,  la infección por Covid-19, para de ellos aprovechar los antígenos, de que puedan ser portadores, para de ello inmunizar a la población que se pueda considerar, en principio de mayor riesgo.
Ahora vamos a proponer, del “curado”, el que se puedan implantar, de aquellos, tejidos compatibles, como si de un trasplante se tratara y así, además de elementos formes diversos, como ejemplo: elementos de médula ósea, y de la misma manera, implantar folículos pilosos de sobrevientes al Covid-19, en población de riesgo. Pensando que tanto la medula ósea, como los folículos pilosos, etc.,podrían actuar,  inmunizando al paciente receptor.
No procede quizás extendernos más, solo que sirva la presente, para consideración, de que el empleo de diferentes tejidos y quizás órganos ( riñón, bazo, etc,) del curado de la viriasis, y no sólo sus fluidos (sangre compatible, orina, LCR…),podría desarrollar inmunología, en receptores seleccionados.

Quizás las mascarillas, tan necesarias desde el punto de vista sanitario,  para prevenir contagios propios y a terceros, han podido venir a usurpar,  lo que también puede ser, para los mismos efectos,  el que se asienten, en una cara inmaculada o lo que es lo mismo, para lo que nos ocupa y es, que su higiniciacion meticulosa, es fundamental, para reforzar la defensa individual y colectiva.
Lavarse la cara, en los tiempos que corren, es imprescindible, nos han enseñado como, se quitan y se ponen las mascarillas, pero que yo sepa, nadie nos ha enseñado, a cómo debemos lavarnos la cara actualmente, cuando y en qué circunstancias.
Habrá que hacerlo, como siempre al menos al levantarnos y al acostarnos, pero ahora, todas las veces que haga falta y no solo pasarnos un agua alojada en el cuenco de nuestras manos, sino aseando también, nuestra cavidad bucal, con un cepillado cuidadoso de la cavidad y además, sonando nuestras fosas nasales, primero un lado y luego el otro y también, nuestras órbitas e incluso, si las circunstancias lo aconsejan, un enjabonamiento de nuestro cuero cabelludo y cuello, en fin, al buen entendedor, con pocas palabras, le basta….

Arrugas y Covid-19

Nos ha venido a la cabeza, sin pretensiones previas al respecto, pero sí pensando, porque las personas mayores, son más propensas al padecimiento de la enfermedad, arriba titulada y entre otras causas, ya incluso conocidas, no solo desde el conocimiento de los Profesionales de la Medicina, sino a través de los Medios de Comunicacion,  etc., queremos aportar, por nuestra parte, como quizás las arrugas de la piel, pueden significar, un campo favorable para los virus.
Las  mismas, al convertirse, en cobijo fácil, para anidamiento, permanencia e incluso, por la mayor superficie de anidamiento que indudablemente representan,  para estiramientos y toqueteos manuales.
También por ejemplo, las manos propias o del personal de cuidados,  etc., podrían impregnarse a su vez, más fácilmente, favoreciendo las capacidades difusoras, de la enfermedad por covid-19.
Ahora solo, lo dejamos como apunte, para investigar.
Añadir que encima, si se les coloca o llevan mascarillas, de una forma poco cuidada, etc., pueden hacer,  que el campo de cultivo, por ejemplo facial y de otros corporales,  sean idóneos,  a un más, para su desarrollo y capacidad infectivo-contagiosa. autónomamente y para terceros….

Parecería, que pare ese «futuro»  Transplante CRANEO FACIO CERVICAL, es tal vez razonable, que sean las zonas basales del Sistema Neurológico Central del Donante, las que contacten anatómicamente, con las más altas, del Sistema neurológico, cervical del Receptor.

Con intenciones investigadoras y habiendo observado, no recordar dentro de la abundante información científica, que sobre el Covid-19 existe, no aparezca atención alguna, en al tema de la Altura,  de los individuos con coronavirus y su posible mayor capacidad, para difundir la afección, dentro de ciertas circunstancias, claro.

Dicho esto, nos proponemos investigar o difundir el tema, para que se estudie y de los resultados, tomar las medidas pertinentes.

Las mujeres y los niños y claro los hombres de baja estatura, podrían ser más susceptibles a la infección. El aerosol virico,  caería sobre ellos,  de forma más contundente, quizás.

Podría ser un medio, con gran capacidad difusora, barato y que claro requeriría, sobre todo, si se emplea fuera del ambiente familiar ( y en casos específicos, de estos últimos) el saber su historial clínico y posibles patologías, para no trasmitir enfermedades indeseables.
Los besos cultivados, de pacientes, que han sobrepasado el coronavirus-19 a su vez, se podrían procesar en laboratorio, para intentar aumentar sus capacidades inmunológicas, y transformarlos en verdaderas vacunas.

Upper maxillectomy The upper maxillectomy yields anterior access to the posterior part of the central cranial base that is limited by the Eustachian tube superiorly and the hard palate inferiorly, even after the pterygoid process is removed (Fig. 5, N–P). Retracting the ipsilateral pharyngeal wall to the opposite side with division of the Eustachian tube produces somewhat limited access to the clivus and C1. Downloaded from https://academic.oup.com/neurosurgery/article-abstract/46/6/1416/2925972 by Universidad de Zaragoza user on 02 January 2020 1449 Intracranial stage Upper maxillectomy Combining the upper maxillectomy with a frontotemporal craniotomy provides intradural access to the anterior and middle cranial fossae, the frontal and temporal lobes, and the basal cisterns by the subfrontal, pterional, pretemporal, and subtemporal routes (Fig. 5, L and Q). Removal of the greater sphenoid wing and floor of the middle cranial fossa opens the superior orbital fissure, foramina rotundum, ovale, and spinosum, and it accesses the lateral wall of the cavernous sinus (Fig. 5, M–R). Drilling the base of the pterygoid plate exposes the pterygoid canal inferomedial to the foramen rotundum. Entry to the sphenoid sinus is obtained by drilling its lateral wall between the ophthalmic and maxillary nerves, or by drilling the anterior part of the root of the pterygoid process above the pterygoid canal. However, the space between the pterygoid canal and foramen rotundum is limited (Fig. 5S). Continued extradural dissection to the posterior part of the middle cranial fossa exposes the anterior surface of the petrous temporal bone, the trigeminal ganglion in Meckel’s cave, and the greater and lesser petrosal nerves in their grooves on the floor of the middle fossa. Drilling the apex of the petrous temporal bone behind the petrous carotid with opening of the dura accesses the anterolateral aspect of the upper brainstem, although the exposure is very limited. Anterior transposition of the petrous carotid is required to reach the central part of the clivus from this lateral exposure. The lateral access is best suited to exposing lesions at the petroclival junction rather than those centrally located in the clivus. Lower maxillectomy The lower maxillectomy, with removal of the clivus and anterior elements of the upper cervical vertebrae, provides reasonable intradural access to the front of the pons, the medulla, and the cervical spinal cord above C4 as well as the basilar and vertebral arteries (Fig. 4, P–S). The vital structures that provide the lateral limits to the extradural bone removal and the intradural exposure include the petrous and intracavernous carotid, especially the artery on the ipsilateral side; the abducent nerve in Dorello’s canals, particularly the contralateral nerve located at the anterosuperior end of the petrous apex; and the hypoglossal canals and occipital condyles. Opening the basilar venous plexus, which crosses behind the upper clivus and the posterior wall of the sphenoid sinus, may result in profuse hemorrhaging. DISCUSSION Among the various anterior routes to the central cranial base, the route most frequently selected for lesions involving the lower clivus and adjacent vertebral bodies has been the transoral approach (12,13). The upper and middle portions of the clivus are also accessible by the Le Fort I transverse maxillotomy, and the additional median section of either the hard or both the hard and soft palates increases the clival exposure, although Cocke and Robertson (9 ) and Cocke et al. ( 10) conclude that the unilateral maxillectomy provides a more extensive exposure than some bilateral approaches, such as the Le Fort I (1,4,23,28). The hemimaxillectomy approach described by Hernández-Altemir (16) accessed both the transmaxillary and transoral routes. Subsequent modifications and extensions have provided added exposure of both the central and the lateral cranial base, permitting en bloc excision of large neoplasms by selecting and combining the osteotomies on the basis of the extent of disease (7,8,10,15,18,19,21). The osteotomies for completing the upper and lower maxillectomy approaches are divided into four basic units: maxillary body, orbital rim, hard palate, and zygomatic arch; and three extended units: coronoid process, pterygoid process, and frontotemporal craniotomy (Fig. 3). The maxillary sinus is a core space for these approaches through which the retromaxillary area can be reached without violating any vital structure, although the transantral route alone provides very limited exposure (11). Removal of the medial orbital rim provides access to the ethmoid and frontal sinuses, cribriform plate, and the anterior face of the sphenoid sinus through the medial orbital route as in the lateral rhinotomy or medial maxillectomy approach; however, temporary sectioning of the medial palpebral ligament and the nasolacrimal duct usually is necessary (27). If the osteotomy involves the lower orbital rim and floor, the infraorbital nerve must be transected. A palatal osteotomy combined with a cut through the maxillary body below the orbital floor enables transmaxillary access to be combined with transoral access, and increases the anterior exposure of the central cranial base. Zygomatic arch osteotomy facilitates the lateral exposure of the upper part of the infratemporal fossa and the middle cranial fossa by allowing reflection of division of the temporalis muscle. Transection of the coronoid process opens the lateral aspect of the infratemporal fossa and allows early exposure of the maxillary artery for control of bleeding, which is common during the maxillary osteotomy. The sphenoid pterygoid process separates the central from the lateral cranial base and blocks anterolateral access to the central cranial base. Removing the pterygoid process provides exposure extending from the central to the lateral cranial base and allows for wide anterolateral access to the clivus and upper cervical spine. The frontotemporal craniotomy, when combined with an orbitozygomatic osteotomy and removal of the floor of the middle cranial fossa, provides lateral access to the cavernous sinus, the sphenoid sinus, and the petrous apex extradurally, and the frontal and temporal lobes and the basal cisterns intradurally

How to arrange and conduct a successful CME event on airway management

ABSTRACT

Medicine is an ever-evolving branch of science, which requires regular teaching and training for the core purpose of patient safety. Physicians around the world are attending newer courses, workshops and continuing medical education (CME) programs to enhance their individual clinical skills. These courses offer much beyond the didactic lectures and are now routinely recommended by the regulatory authorities of most of the countries. This article will provide in-depth information for the conceptualization, planning and conduct of any educational medical course with a special reference to airway management. PREPARATION Academic program preparation: Academic or scientific schedule is the actual soul of the CME and workshop, therefore it should be attractive, with hot topics and with renowned speakers. Academic schedule for the course is decided by a core group of organizing team, comprising of senior anesthesiologists with sufficient teaching and working experience in the field of anesthesia. Academic schedule is usually divided into thematic sessions; Morning session of about 4 hours should comprise of keynote lectures, ‘how-I-do-it’ problem based case discussion, video session showing different approaches to airway management using various gadgets and techniques, and Afternoon session of 4 hours including hands on workshop on various workstations. Keynote lectures are allotted to senior academicians who have extensive experience in the field of airway management. These lectures should cover topics like ‘Airway Assessment- Current Status’, ‘Difficult Airway Guidelines’, ‘Ultrasonography (USG) of Airway’, Apneic Óxygenation’, and ; Extubation of Difficult Airway’. ‘How-I-do-it’ – Problem based case discussions are short case based scenarios of difficult airway; on topics which include airway management in morbidly obese, burns, pediatric cleft lip palate, head and neck trauma, temporo-mandibular joint ankylosis, upper airway malignancy, and pregnancy. For this session practical approaches to particular cases are discussed with special take home message content. The final session before lunch may be planned as a video session which showcases the tricks and special paper 320 ANAESTH, PAIN & INTENSIVE CARE; VOL 23(3) AUGUST 2019 techniques of multiple alternative approaches to difficult intubation like submental intubation, retrograde intubation, lightwand assisted intubation, blind awake nasotracheal intubation, percutaneous cricothyroidotomy and awake fiberoptic intubation. Post lunch afternoon session of the course may be planned for the interactive hands on workshops for participants at multiple workstations. Workstations (Table 1) provide an opportunity to the participants to be acquainted with different options available for airway management, from easier to the most difficult scenario, according to the Guidelines of Difficult Airway Society (DAS). The total duration of the CME + workshop may be from 0900 hours to 1800 hours in the evening (Total content hours of 8 hours + 1 hour break) in a day. Guest Faculty: We must aim to complete the scientific agenda with confirmed speakers. All official invites to the respective faculty members are sent 45 days before the course, so that they have ample time to prepare their talks. Faculty for the course may be selected from the host institution or other reputed national institutions. Some visiting international faculty can be an advantage. All the faculty are reminded from time to time and are requested to strictly comply with their allotted time for the talks. To avoid lapse in the program, few potential faculty from the organizing committee itself are kept in reserve and are asked to prepare a topic to be presented in case of failure of an invited faculty to attend the event. Announcement & Delegate Registration: A successful academic course or workshop requires a decent number of participants. Endorsement by professional bodies can be helpful, but dissemination of an impactful poster or brochure with original content of scientific agenda with renowned speakers on all available venues is required. To get maximum participation, details of the course need to be widely circulated in nearby hospitals, medical colleges and institutions using both print and electronic media. The course is also publicized through institutional website and electronic media through e-mails and WhatsApp messenger. Submental oro-tracheal intubation was developed in order to avoid the need for tracheotomy and to permit unfettered access to the oral region. This type of intubation is done (a) in patients with comminuted fracture of the midface or the nose, where nasal intubation is contraindicated, (b) inpatients who require restoration of the occlusion, and (c) patients whose condition permits extubation at the end of surgery [78]. The neck pathology and normal anatomy can be easily diagnosed using 3-D reconstruction of this submental region to prevent complication of submental intubation.

(1) -As a student in the Complutense University of Madrid, in the Bachelor of Medicine and Surgery, we were among other disciplines: Internal student

of General Pathology, in the Chair of Professor Bermejillo, for later, in the same condition, three years, in the Internal Medicine, of Professor Dr. Don Vicente Gilsanz.

We saw and explored patients, in the Consultations, where our Professor was, Dr.Descalzo, who collected our Histories and Clinical Explorations, for later,

If the clinical case, it was interesting, upload them to the Session of Don Vicente and of course, we tried to go there, whenever we could.

On one occasion, Professor Gilsanz, said in his Session, this boy, has described, Cushing’s syndrome (was a server … ..).

Review that every Saturday, we celebrated, most of the Interns and on our own initiative, our own Session, of Clinical Histories and of truth, that was worth it

Not to say, of course, that we also attended the sessions of the Chair, in the Aula Magna, of the Faculty of Medicine of Atocha, which were magnificent and good, it is not for nothing, but once, we dared to say things and the partner said, say it, say it …

Also to say, that the Stomatology Degree, we did at the same time, that of, by Opposition, Pediatrics and Childcare Competition (to not abandon … the general medicine … ..).

(2) – Now for not going further back, we have been observing, that the issue was not as before and I explain:

The patient arrives, you have him sign the Data Protection and you start to Historian … (still us, on paper, as always …) and it turns out, that as I am, Stomatologist and Oral and Maxillofacial Surgeon, … of course, we see, among others , many dental processes and … .., is not uncommon, that the patient and even direct or indirect close friends, before routine questions, tell you and all that, …, what for ?, if I come, so that I look at the tooth … ..I do not know, if I need to, comment on the subject of the Computer, which covers the patient and part of the relatives ………

All this and of course, much more, it made me think, we have to protect the Clinical History, updated and unintentionally, zas: Well, it came to me to propose it, for World Heritage and in that we are, for a few dates … .

(1) -Ya de Estudiante en la  Universidad Complutense de Madrid, en la Licenciatura de Medicina y Cirugía, fuimos entre otras disciplinas: Alumno interno
de Patología General, en la Cátedra del Profesor Bermejillo, para más tarde, en la misma condición, tres años, en la de Medicina Interna, del Profesor Dr. Don Vicente Gilsanz..
Veíamos y explorábamos pacientes,, en las Consultas, donde nuestro Profesor era, el Dr. Descalzo, que recogía nuestras Historias y Exploraciones Clínicas, para luego,
si el caso clínico, era interesante, subírselas a la Sesión de Don Vicente y como no, a las mismas intentábamos acudir, siempre que podíamos.
En una ocasión, el Profesor Gilsanz, dijo en su Sesión, este chico, ha descrito, el Síndrome de Cushing (era un servidor…..).
Reseñar que todos los sábados, celebrábamos, la mayoría de los Internos y por propia iniciativa, nuestra propia Sesión, de Historias Clínicas y de verdad, que merecía la pena
No dejar de decir, claro, que asistíamos también, a las Sesiones de la Cátedra, en el Aula Magna, de la Facultad de Medicina de Atocha, que eran magníficas y bueno, no es por nada, pero alguna vez, nos atrevíamos a decir cosas y el compañero decía, dilo, dílo…
También decir, que la Licenciatura de Estomatología, la hicimos al mismo tiempo, que la de,  por Concurso Oposición, de Pediatría y Puericultura (parar no abandonar….la medicina  general…..).
(2)- Ahora por no ir más atrás, venimos observando, que el asunto, no era como antes y me explico:
Llega el paciente, le haces firmar la Protección de Datos y empiezas a Historiar…(todavía nosotros, en papel, como siempre….) y resulta, que como soy, Estomatólogo y Cirujano Oral y Maxilofacial,…, claro, vemos, entre otros, muchos procesos dentales y….., no es infrecuente, que el paciente e incluso allegados directos o indirectos, ante preguntas rutinarias,  te diga y ¿todo eso,…, para que?, si yo vengo, para que me mire la muela…..No sé, si necesito, comentar el tema del Ordenador, que tapa al paciente y a parte de los familiares………
Todo esto y claro, mucho más, me hizo pensar, ¡hay que proteger a la Historia Clínica, actualizada y sin querer, zas: pues me vino el Proponerla, para Patrimonio de la Humanidad y en eso estamos, desde hace, pocas fechas….

Abstract

This is a comment on a prior letter regarding authorship1 that appeared in the June 1994 issue of the Archives regarding an article by Catalano and Biller.2

The original English description of the extended maxillotomy procedure was by Hernandez-Altemir of Zaragoza, Spain.3 He had previously published this technique in a Spanish journal in 1982 and 1983.4,5 A scholarly review of all the world’s literature would have turned up this article by Hernandez-Altemir. Maybe then it would not have been reintroduced as a new procedure by three different studies.1,6,7

This episode brings to mind several problems that are becoming more apparent to readers of the otolaryngology literature. Some of it may be related to our increasing reliance on computer literature searches. If the key words used in searches do not match with similar articles, these articles will be omitted in the search results. I think this

Quizás la Cirugía Transfacial y la Craneofacial Pediculada, pudo iniciarse si el lector es curioso, mucho antes de 1982, cuando en ESTOMA, Volumen II, Num. I, Páginas 41-48, 1982, dimos a conocer: “Un caso de Angiofibroma Juvenil Gigante de Cavum Intervenido por la Técnica de Desarticulacion Temporal Pediculada a Mejilla del Maxilar Superior” y sobre lo mismo, también en ESTOMA, Volumen III, Num. I, Páginas 75-82, 1983, con el entonces Titulado: Desarticulación Temporal Pediculada a Mejilla del Maxilar Superior(es) como Via de Abordaje Transfacial a las Regiones Fundamentalmente Retromaxilares y para otras Indicaciones (Via Maxilopterigoidea).Una nueva técnica. Y un poco más reciente, en el Ilustre Colegio Oficial de Odontologos y Estomatólogos de Aragón, Symposium 24 de febrero de 1995 Zaragoza, Cirugía Craneofacial Pediculada, un Nuevo Método, para luego Presentarse en el XIII Congreso Nacional de la Sociedad Española de Cirugía Oral y Maxilofacial en Pamplona-Iruña 29-30-31 Mayo- Donostia-San Sebastian 31 Mayo – 1-2-3 junio 1995 y en:The XIV Congress of European Association for Cranio Maxillofacial Surgery, Helsinki 1-5 September 1998.

A veces la iluminación, por causas muy diversas deja de ser la adecuada y al respecto, La Luz del móvil, puede ser una solución más o menos temporal, en este caso en Microcirugía Experimental. En las imagenes, se pueden ver las diferencias durante la disección de los vasos y otras estructuras.!
